2025 CVHS SENIOR PROM

Information & Policies Regarding Prom

1. You must use your student ID as the access code when purchasing tickets
2. You must include your 3rd hour teacher's last name when purchasing a ticket. If you are not in the building for 3rd hour, input "Sacco". 
3. Your tickets will be delivered to your 3rd hour class the week of May 12th 
4. A maximum of two tickets can be purchased per senior. One for yourself and one for a guest if you choose to bring one. Guests must be a minimum of a junior in high school and a maximum of 20 years old. You must purchase both tickets at the same time. Your student ID will only work once as an access code.
5. Prom tickets can be purchased April 14th - 25th.
6. You need to submit a table form by May 2nd (see link below)
7. Picture ID of outside guests ONLY must be submitted when purchasing their ticket. Picture must be of a school ID or license. 
8. All school rules and policies must be followed during this event
9. The ticket is non-refundable and non-transferable
10. The ticket holder must be twenty years of age or younger
11. Ticket and photo identification must be shown at prom for admission
12. No smoking is allowed inside or outside Zuccaro's Banquet Hall. This is a township ordinance
13. Alcoholic beverages/illegal drug consumption will not be tolerated. Police officers and security will be present at prom
14. Appropriate, formal apparel is required
15. Dinner will begin promptly at 7:00 PM. Students must be seated at their reserved table during the entire dinner.
16. Students will not be allowed entrance past 7:00 PM and will not be allowed reentrance if they leave early

• Only ONE person from your table needs to fill this form out 
• There is a maximum of TEN people at a table
• You do NOT need to fill your table with ten people. If your table has less than ten people, other guests may be placed at your table to fill the table.
• Forms must be submitted by May 2nd

Outside Guest's Picture ID
